How can EHF help your baby?

Infants with cow milk allergy cannot tolerate proteins found in regular infant formula because the proteins are too large. In an extensively hydrolyzed formula, these proteins are extensively broken-down, making them hypoallergenic and less likely to cause an allergic reaction.

Cow Milk Allergy or Lactose Intolerance?

Differentiating between cow milk allergy and lactose intolerance can sometimes be confusing, since both might share similar symptoms. It is important not to confuse the two.

Cow Milk Proteins that Trigger an Allergy

Two types of cow milk proteins can trigger an allergic reaction in babies allergic to cow milk. These are whey and casein and babies may be allergic to one or both. These proteins trigger the body’s immune system causing it to overreact. When the immune system is affected, it can lead to a wide range of symptoms that can occur in different parts of the body.

Lactose Intolerance

Lactose intolerance does not involve the immune system. It is the body’s inability to properly digest lactose, a natural sugar found in cow milk and other mammalian milks. Lactose intolerance occurs when the small intestine fails to produce enough of the enzyme lactase which is needed to digest lactose.

Breastfeeding or breast milk

For babies who are diagnosed with cow milk allergy and are breastfeeding (or receiving breast milk), speak to your healthcare provider about the need to do a maternal elimination diet. This is where dairy is removed from your diet, always under guidance from a doctor or dietitian. Supplementing your diet may be required to help ensure adequate intake of nutrients (notably, calcium) found in dairy products. In some cases where breastfeeding is not possible or there is not enough breast milk, a hypoallergenic infant formula may be required.

Hydrolyzed Infant Formula

Hydrolyzed infant formula is made with proteins that have been extensively broken down (or hydrolyzed) into smaller chains or peptides. This makes the formula easier for infants with cow milk allergy to digest1. Whey and casein are the two most common proteins used in hydrolyzed formula. Also, hydrolyzed formula can be either partially or extensively hydrolyzed depending on the degree of hydrolysis.

Partially Hydrolyzed Infant Formula (pHF)

pHF is made with cow milk proteins that have been partially broken down into short and long peptides. This type of formula is not hypoallergenic and may still trigger an allergic reaction in babies with cow milk allergy. For this reason, pHF is not recommended for the dietary management of cow milk allergy.

Extensively Hydrolyzed Infant Formula (eHF)

Unlike pHF infant formula, the proteins in extensively hydrolyzed infant formula (eHF) are extensively broken to reduce the risk of causing an allergic reaction. When the protein in infant formula gets broken down into small enough chains, the baby’s immune system no longer recognizes the cow milk protein, and most times there’s no allergic reaction in babies with mild-to-moderate cow milk allergy. This is why eHF is considered hypoallergenic and can be used for the dietary management of cow milk allergy.

Amino Acid-Based Infant Formula

Amino acid-based infant formula is made entirely of free amino acids. This type of formula is the most hypoallergenic type of infant formula and is unlikely to cause an allergic reaction even if your baby has a severe cow milk allergy. Hypoallergenic amino acid-based infant formula is designed to help with the dietary management of cow milk allergy and other food related allergies.

Other Types of Infant Formulas

If your child has been diagnosed with a cow milk allergy, your healthcare professional will prescribe a specialized hypoallergenic infant formula. The type of infant formula your baby will need depends on the severity of their allergy. In severe cases of cow milk allergy, an amino acid-based infant formula may be required.

Soy-Based Infant Formula

Soy-based infant formula is made with soy protein and is an alternative to cow milk-based infant formula. However, up to 35% of babies with a cow milk allergy may also react to soy and for this reason, soy-based infant formula may not be suitable to manage cow milk allergy.

Studies done in Europe and the US have shown that CMA may persist in as many as 50% of children at 1 year of age, with < 25% remaining allergic at 3 years and some persisting into early teenage years.

A cow milk allergy (CMA) is an immune system response to one or more proteins found in cow milk and products containing milk. These proteins trigger the body’s immune system and cause an allergic reaction.

Preparation and Storage

Important: Powdered infant formula is not sterile. Formula should always be prepared immediately before feeding. Do not boil formula, and do not use a microwave to prepare or warm formula. All Aptamil® products come with preparation instructions. Please see packaging for further information.

BECAUSE POWDERED INFANT FORMULAS ARE NOT STERILE, FAILURE TO FOLLOW INSTRUCTIONS PROVIDED MAY MAKE YOUR BABY ILL.

  1. Wash hands with soap and sterilize all utensils according to the manufacturer’s instructions.
  2. Boil 1 litre of fresh water. Cool water for 30 mins and no longer. Measure the required amount of warm water (refer to feeding guide) into a sterilized bottle. Be careful of scalding. Do not use artificially softened or repeatedly boiled water to prepare formula.
  3. Using the scoop provided, level off the powder with the back of a clean, dry knife and scrape off extra powder. Do not press/heap the powder.
  4. Add the correct measure of powder to the water. Place the sterilized teat and cap on the bottle and shake
    vertically and vigorously for 25 seconds to dissolve the powder.
  5. Cool under running tap and check temperature of feed. Feed immediately.

Recommended feed concentration: One level scoop (as provided in the can) provides approximately 4.5 g of powder. Add one level scoop of powder to every fluid ounce (30ml) of water to yield the recommended feed concentration.

Storage
Store unopened cans at room temperature. Avoid extreme temperatures. Cover opened can with lid provided and store in a cool, dry place. Once opened, formula must be used within four weeks.
